Document Submission

For any documents that includes Personally Identifiable Information (PII), we will only receive them through Bankruptcy Documents.  Any other manner of receipt will be deleted automatically from our system.

 

Please only upload this proof through Bankruptcy Documents in the correct drop down.  if you do not see a drop down that correlates with what you want to provide, then do NOT just attach it anywhere you decide and change the description.  If you do not see a drop down specifically identifying what you are trying to upload, do not attach it and ask an attorney if it is needed.  If we want it, we will let you know to send it to us directly.

We will not accept mailed, faxed, hand delivered or any other manner of receipt of PII directly from the Debtors.  All must be submitted through the attorneys office.

 

Please make sure all attachments that are uploaded into Bankruptcy Documents are:

• Complete – with all schedules and other forms attached and in proper order do not attach a debtors W-2 form or 1099 by itself.  They should be filed only as the attachments to a copy of the full return.

• Legible – if you can’t read it, neither can we.

• One complete document.  We will delete any upload of numerous individual documents which should be uploaded as one document.

• Pay advices, put the items in date order for the specific debtor and upload as 1 items.

• Profit and Loss statements, put the items in date order for the specific debtor and upload as 1 item.

• Bank statements, for each separate bank account, put the items in date order for the specific debtor and upload as 1 items.

If an attorney has requested an item and there is no upload category, then you would send it by email to the attorney who asked for it or  attorneys@ch13jaxfl.com.
